It's there to keep things aligned and prevent direct metal-on-metal grinding, which would chew up much more expensive components pretty fast. Think of it as a sacrificial layer that takes the abuse so your main structure doesn't get hammered.

When this PAD starts getting thin, or if it breaks up, you’ll usually notice a few things. First off, you might hear some new squealing or grinding noises from the feed system – that's the metal-on-metal contact starting. You could also see the feed beam or drill string start to bind up a bit, or maybe it just doesn't track straight anymore, leading to uneven wear on your drill string components. Sometimes, if it's really gone, you'll feel increased vibration through the rig, especially during collaring or when pulling rods.

Replacing these isn't usually a huge job, but it's one of those things you want to catch before it causes bigger headaches. When you're in there, always check the mating surfaces for any gouges or excessive wear that might have happened before the PAD completely failed. Also, make sure all the mounting hardware is still good and torqued correctly; a loose PAD is almost as bad as a worn-out one, and it's easy to overlook those bolts.
